Parents/Guardians are asked to complete the following forms during the first 2 weeks of the school year.

3. Medical Information Sheet (Print the file and bring to office if your child has medical condition) ( )- Required
4. Windermere Field Studies Neighbourhood Consent (Will be distributed to students - digital copy below) - Recommended
